Patient Letter of Discontinuation of Services

Effective Date: October 29, 2025

Dear Patient,

Caring for you and your family has always been a privilege for us here at Bravely.

That is why sharing this update weighs heavily on our hearts.  After much prayer, discernment, and careful evaluation, our Board of Directors and leadership team have made the difficult decision to permanently end NaPro technology, prenatal, and pediatric services at Bravely. This change will take effect as of December 31st, 2025, or at the end of our insurance obligations, whichever is later.

While these programs have been deeply meaningful and impactful, they are not sustainable with our current resources.

We are committed to ensuring that all patients experience a smooth and compassionate transition of care.  

  • Bravely is no longer accepting new NaPro, prenatal, or pediatric patients. New patient appointments will be canceled.  
  • Our goal is to help current patients transfer their care to new providers as soon as possible or before December 31, 2025.
  • Our team will be reaching out directly to assist in rescheduling, canceling, or transferring upcoming appointments.
  • The midwifery/homebirth program will continue through April 2026 for patients already scheduled as communicated earlier this year.


It is very important that you continue to receive medical care.  To ensure your medical care continues without interruption, we encourage you to establish a relationship with a new healthcare provider as soon as possible.

A copy of your medical records will be transferred to a provider of your choosing upon your written authorization. Your records are confidential, and we will not release them without your permission. Your medical records will remain on file with Bravely Women’s Health in accordance with medical record retention requirements.

  • To request your records, please complete a Medical Records Release Form, available by calling or texting our office at 765-742-1533, by April 30, 2026.
  • After April 30th, 2026, please call Bravely to determine the new records process or to see if a different medical records custodian has been assigned.


While these programs are ending, Bravely will continue offering free services which include:  

  • Pregnancy testing, ultrasounds, and pregnancy options support
  • Well-woman exams
  • STI testing and treatment
  • Abortion Pill Reversal  
  • FEMM
  • Advocacy and tangible needs assistance (clothing, diapers, wipes, formula, etc.)
  • Post-abortive and pregnancy loss care
  • The Every Mother’s Advocate Program (crisis to stability)
  • I Decide for Me education in local schools
