It evaluates the overall health of an individual and identifies any potential risks or underlying conditions that may be present. The goal of the physical is to ensure athletes are safe while participating in their chosen sport. The IHSAA requires a physical on file with the school for the athlete each school year. This is billed through your insurance or we accept cash pay. This is not a free service.

We are located in the heart of Lafayette, IN – ten minutes from Purdue University. We are at 938 Mezzanine Drive, just off Park East Blvd between McCarty and 26/South St.
Most of the time we do by appointment only. However, if you are pregnant and considering abortion, we will see you the same day for your pregnancy confirmation and abortion education.

Depending on how far along you are, we may have you come into our office first for a pregnancy confirmation. After we confirm your pregnancy, we will schedule you for an ultrasound.
We really care about the women we converse with and respect their decision. However, since more than 90% of women come to regret their decision to abort, and the fact that we cannot attest to the safety and cleanliness of any abortion clinic, we remove ourselves from the referral process. If you would like, we can provide you with a list of screening questions to ask the clinics you call that may help you to identify some red flags. We do also commit to being here for clients after the procedure if they have concerns or difficult emotions to work through.
Our center acts as a neutral party who can confirm the viability of pregnancy and educate you on all the pregnancy options available to you. Ultrasound is an important step in your abortion choice. You may not be aware, but roughly 25% of early pregnancies end in miscarriage. Ultrasound would confirm viability, meaning the pregnancy looks as if it will carry on. This protects you from having to pay for a procedure you may not even need. An ultrasound can also give you exact dating of your pregnancy so that you do not choose a procedure that may not be correct for the gestation of the pregnancy. We also help our clients sort through their emotions about the pregnancy, identifying what obstacles they face and connect them with resources they might need to move forward. We do not provide chemical or surgical abortion at our center, but we provide the first steps necessary before your abortion (ultrasound and abortion education).
We take a urine sample and test for the presence of a hormone called HCG (human chorionic gonadotrophin). You can get your results within 5 minutes.
You can bring one adult with you to your appointment.
We provide limited obstetrical ultrasounds to confirm viability and dating of your pregnancy. During this ultrasound, if we observe gender that is a bonus. If you are currently in prenatal care and experiencing any concerns with your pregnancy, please contact your prenatal care provider.

At Bravely we want you to have a good experience when you get an ultrasound. We start with an abdominal ultrasound and if you’re under 10 weeks we also do a vaginal ultrasound. We ask that you come with a full bladder. We will walk you through each step of the ultrasound and explain it as we go. This is interactive as we want you to ask questions and learn about the development of your baby. We give you photos of your baby at the end of the appointment.
Well-woman exams are for patients over 21-years-old. This will involve a pelvic exam and pap smear with one of our medical providers. They do this so they can test for cervical cancer. This will also involve going over your health history, performing a breast exam, and getting your vitals. We can also test for STI’s during this time. We will also answer any questions you may have.
We test for most every STI as long as it is indicated. We can test for Gonorrhea, Chlamydia, Trich, Syphilis, HIV, and sometimes herpes if there is a sore present. We can also test for Bacterial Vaginosis and yeast but those aren’t considered STI’s. We can also draw blood for hepatitis panel if indicated.
